We have an errant application somewhere, but don't know how to determine what application is causing this to happen. Any suggestions on where to look?

Can you narrow down the problem to a date, to see what software was installed before? Maybe your AntiVirus is causing the problem, try do dsiable it and see what's happen. What are the hardware specs of the server?
