I had this happen with a StarTech USB2.0 PCI adapter card. If I inserted a USB flash drive or tried to load a regular CD or DVD into a USB DVD-RW drive the entire PC would deadlock (both Win2K and a Linux live CD). Up until recently it would read UDF formatted CD-Rs OK and would burn DVDs and CDs, but it has now packed it in completely.
