delete duplicate apps on Startup menu

HI, how do I delete duplicate apps on Startup menu, not just to slide "off."The app "practice works server" supposed to be a startup app but it did not work at startup, IT technician manually replace it in the startup folder several times, (probably the reason why it appears three times on the list.) The app still does not work at start up most of the time. We look at startup folder, registry, did not find duplicates, it only appears on this list/menu three times, how do we delete the two inactive ones from the list.Your help is much appreciated.
