Defragmentation and how soon to defrag again?

 Once you do a defragmentation how soon will you need to do it again my computer seems like it needs it quite often. Also I checked it like the next day and noticed something about the files that can not be moved. It looked like a fragment in the blue

was green instead of red is that possible? When I was running the defragmentation I was watching the process it looked like there was green fragment every now and then in the blue.
