Application icon in VS2008 (Express) not getting all icon sizes

When I made a new icon for this small application that I am writing, when I added an icon to the project, not all of them get into the executable.* Approximately between Small and Medium icons, instead of my icon, I see a blurry blue and white window icon, and I made sure that my icon had the 256x256 PNG-compressed, 96, 64, 48, 40, 32, and 16px icons included.** What could be causing this?
