You can "Forward" the email as proof to the second insurer if they will accept this and give you their email address.
There should be an option to do this from your email inbox.
Don't forget to put your quote reference on the forwarded email.

If nothing else is a problem - machine running ok - not throwing up error messages etc ......then if you can live with this entry in Device Manager it really is not a problem.

Since my last post I have moved about 80 miles from home for a few days :)
If you wish to read the sfc log then do the following:
Go back to the DOS screen ( Run as Administrator )
Type in the following:
findstr /c:"[sR]" %windir%\Logs\CBS\CBS.log >"%userprofile%\Desktop\sfcdetails.txt"
Hit ENTER
This will put a file on your Desktop called "sfc Details"
If the problem is still there then the SFC log will probably be of no use to us in solving this little dilemma :)

( some cards come with the two sizes of bracket as I have said earlier )

This wasn't the cause of the addition to "Other Devices" was it ?
You only need to see if you can access the drive - then try another USB Port.
Looks as if it is probably hardware related.
When you un-installed it ...and it returned Windows must be finding something.
What is confusing is that it puts ( what it thinks is ) a USB Controller in "Other Devices" and not in USB Controllers at the bottom of the list.
Please confirm that all USB Ports are functioning properly.
If you have an external hard drive try this as it uses more power than a Memory Stick.
This addition to Device Manager is not doing any harm .... just annoying :)

I think you said earlier that you had a Win7 installation Disk ?
If so ...
Get back to the DOS prompt ( Run as Administrator )
At the prompt type .... sfc /scannow ....ENTER [ there is a gap after sfc ]
This will scan the system files for errors.
You may be prompted to insert the Win7 disk.
This checks the Windows System Files for errors and tries to correct any that are found.
Start your machine in Safe Mode.
Switch on and constantly tap F8 about once per second.
Select Safe Mode from the list.
Go to Device Manager again.
Is the problem device still showing ?
If not - it is probably software that is causing the problem.

Is it to the far right of where the screw holds the video card in place ?

That is the correct one for the USB ports.
I assume there is no problem with this.
The other one - under "Other Devices" - is Windows not being able to recognise it.
Why it is saying "USB Controller" I have no idea !!
Uninstalling it should have removed it - but Windows is still finding something that it does not know where to allocate it in Device Manager ...hence "Other Devices"
What happens if you right click on this "Other Device" and click on update drivers ?
Let Windows search for drivers automatically.

Nice of them to let you keep the card.
If you still have the card - install it anyway ( just to test ) and connect up the two monitors.
If it works as we hope - then you can get a more suitable card ....if there are problems then nothing lost :)
So - is Device Manager looking as it should now ? i.e. no yellow exclamation marks or Teredo devices etc ?
( except for USB ...which we know about :) )
Right click on this and delete / uninstall it.
Reboot.
Does it re-appear ?

I have moved your post to this forum - it is probably best to have your own thread.
Can you boot up in Safe Mode with Networking ? [ using the problem machine ]
Switch on and constantly tap F8 about once per second.
You should be taken to the Advanced Startup Options - select Safe Mode with Networking from here.
The graphics will look odd - but this will correct itself when you boot up normally.
See if you can get on line this way.
If you can ....
Download Malwarebytes and run it as described here: click here
There is no need to run the other option ( step 2 ) yet.
If you cannot access the internet this way post back and we will take it from there :)
It may be necessary to move this thread again - to the Malware Removal Forum - I will let you know if this is necessary.
Let's try disabling Teredo Tunnelling
Start > type in .....cmd ..... ( don't hit ENTER yet ) ....right click on the cmd.exe that will appear top left of screen
Click on "Run as Administrator"
At the command prompt type ........ netsh interface teredo set state disabled .....ENTER
It may need a re-boot.
Go back to Device Manager and see if the teredo tunnelling entries are still there.
Also ..
You didn't answer my last question re. Wireless and LAN ( wired ) connections.
Are they OK ?

I have my doubts that ipV6 is needed on your machine - better to leave the Registry alone until we actually need to make changes.
Lets check first to see if ipV6 is enabled.
run this - click here
If it is "Supported" we can look at disabling it. [ ipV4 is what we DO need enabled at this point in time. ]
If it is "Supported" i.e. enabled then take a took here and uncheck it - see the image in step #3
[ TCP/IPv6 needs to be unchecked - then click on OK ]
